The man who has been in charge at Blue Cross and Blue Shield of Massachusetts for the past six years resigned, effective immediately, as chief executive yesterday. Normally, that kind of hasty exit leaves little doubt whether a CEO jumped or was pushed. Killingsworth certainly wasn’t talking about it yesterday.
